I'm fairly certain this is actually the birth certificate for Sophie Dyszkiewicz. How her name was changed from Julia to Sophie is unknown to me. She was the daughter of Lawrence Dyszkiewicz and Mary Mazurkiewicz.
After her marriage to Michael Kramer in 1916, she changed her maiden name to Smith. Sophie and Michael had two daughters, Mildred and Claire Kramer.
